Outreach Request Form

This form will send your request to MOHAI's Education team. This does NOT guarantee your reservation.  MOHAI’s education team will reply letting you know if your request has been accepted within 3 business days.   Reservations are on a first come, first served basis and no trunk reservation is final without a confirmation number.

Groups will not be admitted without adequate chaperones
K-5: One adult chaperone required for every 5 students
6-8: One adult chaperone required for every 7 students
9-12: One adult chaperone required for every 10 students
